QAPI Fundamentals for Long-Term Care

Define Quality Assessment and Performance Improvement, including QAPI principles, and accurately distinguish Quality Assurance activities from Performance Improvement initiatives within a long term care setting.

Analyze facility data sources, select at least three relevant indicators, and apply basic statistical and graphical techniques to identify trends and prioritize improvement opportunities.

Design a measurable Performance Improvement Project using a PDCA or PDSA cycle, specifying aim statement, measures, data collection plan, timelines, and roles for at least five interdisciplinary team members.

Demonstrate implementation strategies for sustaining change, including root cause analysis, process mapping, staff engagement tactics, and evaluation of outcome, process, and balancing measures over a three month period.

By the end of this module, learners will be able to describe the five CMS QAPI elements and give two long-term care specific examples for each element, measured by a scored short-answer assessment.

Learners will construct a SMART aim and select appropriate process and outcome measures for a QAPI Performance Improvement Project targeting falls or pressure ulcers, demonstrated through a submitted PIP proposal.
